Patient Support Programs

Support Beyond Your Treatment

At New Life Dialysis Center, we believe exceptional care extends beyond the dialysis chair. We are here to guide and support our patients through important steps that can improve their quality of life.

Our Commitment

How We Support Our Patients

Living with kidney disease presents challenges that go beyond medical treatment. Our care team is committed to helping patients navigate the resources, programs, and processes that matter most to them and their families.

Transplant Coordination

Kidney Transplant Support

We support eligible dialysis patients throughout the kidney transplant referral and evaluation process.

  • Transplant referral coordination
  • Documentation assistance
  • Communication with transplant centers
  • Patient education and guidance
  • Ongoing support throughout the process

A kidney transplant can be a life-changing option for eligible patients with end-stage renal disease. New Life Dialysis Center works closely with patients who wish to explore transplant as a next step in their care journey. Our clinical team assists with coordination, required documentation, and communication with transplant centers — helping patients understand each step involved in pursuing placement on a kidney transplant waiting list. We are here to support our patients and help ensure they have the information and guidance they need throughout this process.

New Life Dialysis Center does not perform kidney transplants and cannot guarantee acceptance onto a transplant waiting list. Eligibility is determined by the transplant center and the patient's medical team.

Benefits Support

Disability Benefits Assistance

We provide support and assistance to dialysis patients with documentation and information that may be required when applying for disability benefits.

  • Medical documentation support
  • Assistance gathering required records
  • Guidance on the application process
  • Coordination with your care team
  • Compassionate, patient-centered assistance

Navigating the disability benefits application process can be overwhelming, especially while managing a chronic illness. New Life Dialysis Center can provide support and assistance to dialysis patients with documentation and information that may be required when applying for disability benefits through programs such as Social Security Disability Insurance (SSDI) or Supplemental Security Income (SSI). Our team is here to help patients gather the medical records and supporting materials that may be needed as part of their application.

New Life Dialysis Center does not determine eligibility for disability benefits, approve applications, or guarantee approval. Eligibility and approval decisions are made solely by the relevant government agencies.
